2010 Exemption Request to Continue Use of
Social Security Numbers (SSNs)
SSNs and other identity information are valuable and their theft by organized crime and others has become a major issue for higher education. The chancellor has charged UNL to eliminate all non-essential use by January 1st, 2007. Exceptions must have the chancellor's approval.
Instructions
Purpose & Audience
Timely and accurate press releases concerning a potential compromise of your sensitive data is essential for UNL. Should your application become compromised this exemption request will be one of the first documents pulled by University Communications to begin composing a press release. Your justifications in this request must meet the "court of public opinion" that UNL was doing its best to protect privacy and personal information. Please compose your responses with this public in mind.
Request for Exemption
As a department or as an individual, identify all areas where SSNs could be found. This includes servers, desktop and/or laptop computers, staff titles associated with the computers, ex. Processing Specialist (don't need to list individual names), applications, files, etc. A single exemption request for your area may be all inclusive of the aforementioned.
New Applications
New Use of SSN's must be reported. You can either fill out a new request or update an existing request.
Several blanket exemptions exist that you are not required to file an exemption request for. They include the following:
- Direct access to SAP/HR and SIS+ application data which is housed on the Computing Services Network systems. However, if you have SSNs from these systems stored on your electronic devices an exemption must be requested.
- Paper documents required for SAP/HR and SIS+ that are stored in locked filing cabinets and locked rooms; and shredded when no longer needed. However, if you have SSNs from these forms stored on your electronic systems an exemption must be requested.
Annual Renewal
Conditions change so you will be contacted yearly to review and renew your exception requests. The 2010 Renewal Form can be found below.
