Online Request Forms

You may initiate many service requests for network connections, communications services, computer accounts, file recovery, and other services provided by Information Services for the UNL campus and personnel by completing an online form. Please see the following list for available forms.

Network Connections

Departmental Port Activation Request

 

Before requesting a Port Activation, departments must ensure that a network port is both installed and certified. Note: The physical connection to the network is also referred to as a dataport or data jack.

  1. Make sure that a data port is available. Network connections can be identified by a gray jack located in a wallplate (faceplate), generally the same wallplate as the telephone connection. If you lack a network connection, contact Communications to have the port & wires installed: 472-3434 or use the online Add a Data Jack form. (There is a charge for this installation.)
  2. If a data port is available, the second step is to ensure it is certified, meaning that a Jack ID (label) appears on the wallplate. Locate the port's ID number and write it down. If the port is not certified, you need to request Port Certification.
  3. The last step is to request Network Wallplate Port Activation. Note: If you experience any difficulty filling out this form, please request an activation by calling Telecommunications at 472-1109.

Computer Network Card Activation

Each computer accessing the Internet uses a unique Internet Protocol (IP) address to identify it on the network. To assign an IP address, IS Networking personnel need the network card's Ethernet Card Address. Both of the forms below require that number. Please contact your unit support person or the IS Help Desk (472-3970 or 866-472-3970 toll free or helpdesk@unl.edu) for help on locating the card address.

Residence Halls - Student Internet Access

Access to the Internet in the UNL Residence Halls is free; however, you will need a network card, a RJ45 Ethernet cable and software configured on your computer. Please see the UNL ResNet site for more information.

Forms for adding or changing wireless service, students requesting on-campus phone service (or changing optional features in the service), Personal Voice Dialer, and other services.

Departmental Service Requests
Related to wireless service, Visual Messenger sign up, audio conference call sign up, videoconferencing request, Personal Voice Dialer, and other services.

Departmental Telecommunications Service Request Forms
Request add, remove, or change of service for phones (office or wireless), voice mail, pagers, fax machines, UNL calling cards, Internet service, and more.
